Meet Your Goals With An Experienced Partner
Every person’s journey toward safety and stability is different. At Lifebridge, we prioritize your goals and break them down into reachable steps, then partner with you to make progress towards your priorities. Some of the most common goals we help with are:
- Getting an ID
- Applying for housing
- Addressing debt
- Finding and accessing resources
- Getting a job
- Connecting with substance use recovery resources like AA (Alcoholics Anonymous) and attending a meeting
No matter what your priorities are, a Lifebridge case manager will work with you to understand your unique goals and develop an individualized action plan that plays to your strengths.
What is a case manager?
Do you have an end goal but aren’t sure how to start? Have you tried to access a resource you needed to survive but haven’t received it because you met frustrating red tape, complicated paperwork, or something similar?
You’re not alone — accessing even basic services can require complex, interlocking documents and digital access. Case managers are experienced professionals who partner with you on your journey to stability, help you navigate these complicated systems, and advocate on your behalf when needed.

Lifebridge case managers come from different backgrounds, families, educations, and experiences, but we do have a few important things in common. We are all compassionate, hardworking people that see you as a whole person with goals, strengths, and autonomy.
Free Case Management For All
Some may think that Lifebridge only serves unhoused individuals. In reality, we serve anyone in crisis: older adults, families, those facing financial hardship, people fleeing domestic violence, and more.
Many housed people need Lifebridge’s services. If you’re struggling to make ends meet or are one paycheck away from losing your home, getting free help from experienced case managers could be the key to finding the stability, resources, and community you need.
On-Going Support to End the Cycle of Homelessness
Simply housing someone doesn’t guarantee long-term stability; issues such as paperwork errors, substance use disorders, disabilities, or limited support can quickly lead to eviction. To prevent you from cycling back into high-risk situations, we offer you end-to-end, ongoing support even after you meet your goals.
What if things don’t work out?
Setbacks happen. When they do, it’s easy to feel like you can’t talk to your Lifebridge case manager, but staying connected is the best way to work toward safety and stability. We will meet with you to understand your current situation and continue working toward your goals.
